Behavioral Health Home Project (BHHP)
As the Lead Wellness Nurse Navigator, Amanda works to integrate the physical health and behavioral health needs of the consumer who has been identified as a high need service user with concurrent physical health concerns. Since starting work with Chartiers’ Behavioral Health Home Project in the spring of 2016, Amanda Lewis has been busy completing physical health assessments on all the referred consumers and consulting with the Blended Service Coordinators who work directly with individuals who are at risk. Her role is to access and build linkages to community supports and resources as well as enhance coordination and integration of primary and behavioral health care to better meet the needs of people with multiple chronic illnesses. Amanda advocates for her consumers and educates them to take control of their health and wellness goals. She also recognizes and supports the importance of caring for the whole person.
