Celebrating Our Nursing Team at the LTSR

The nursing team at Chartiers’ LTSR includes 4 LPN’s: Diane, Melissa, Pat and Jessica with Sara as the LTSR RN Supervisor. Together they provide nursing services to the 16 residents of the Long Term Structured Residence (LTSR). The program provides for the mental and physical health care needs of a geriatric population. Upon the arrival of new residents, the nursing team completes the required evaluation and assessment for their treatment. The nurses, along with the other residential staff and the treatment team ensure a continuum of care is delivered to support the emotional and physical needs of the residents. The nursing team provides strong communication and organizational skills and can be responsible for communicating with residents, families, medical staff, therapists, colleagues, case managers etc. to promote continuity of care and services for all the residents.
Utilizing their knowledge of psychiatric disabilities and medication management, along with community inpatient procedures, community resources and emergency hospitalization procedures the nursing team are vital to the smooth operation of the LTSR.
The nurses provide compassionate support and understanding to all of the senior residents at the LTSR. For some, who have no other family, the nurses and residential staff become their family. These relationships are built over time and reflect Chartiers Center’s mission of compassionate support and understanding provided by the nursing and residential staff at the LTSR.
