Chartiers Recovery Support Service Line, otherwise known as Service Coordination, extends the support arm of the agency to coordinate the care of behavioral health consumers and those seeking services. For many consumers, maneuvering the behavioral health system can be overwhelming. Service Coordinators are available to work along side individuals and families entering the behavioral health system for the first time, as well as with individuals who have a serious and persistent mental illness. Referrals come from hospitals, the justice system, community providers, crisis intervention services, outpatient providers etc. As the Service Coordinators are also mental health professionals, they have a solid understanding of how to assist consumers with their recovery goals and enhance their quality of life.
Administrative Service Coordination (ASC)
Administrative Service Coordinators work in conjunction with the treatment team to link individuals to community services such as housing, education, vocational training, recreation, advocacy, medical and legal needs. A liaison is maintained with St. Clair Hospital and other community hospitals to coordinate discharge and follow-up services. Service Coordination is available to children/adolescents and adults who require behavioral health services.
Blended Service Coordination (BSC)
Blended Service Coordinators are available for a specified group of individuals who have more intensive and immediate service coordination needs. Blended Service Coordinators help consumers identify their strengths and establish personal goals. Linkage to available community resources, funding, and support services is provided as needed. This working partnership is voluntary for participants and is available 24-hours a day.
