Chartiers is happy to recognize employees for working together as a team to expand the reach of their efforts. Together, Jacque Hunter and Amanda Lewis implemented the Behavioral Health Home Program (BHHP) at Chartiers and have become the gold standard by which other teams have worked to exemplify.
Over the past year, Jacque and Amanda have worked diligently to plan and implement the BHHP program. They educated themselves and others on the wellness coach approach, attended BHHP learning collaborative, presented their findings at the regional conference, continue to meet program outcomes and complete weekly case conferences to support best practice.
The BHHP operates under a “whole-person” philosophy and believes that the integration of physical and behavioral health care services is critical to the achievement of wellness and enhanced outcomes for each consumer. This month the BHHP celebrates their 1 year anniversary!
Maggie Cella is the next Shinning Star!. Maggie was assigned a monumental task to assist in moving Chartiers closer to paperless Medical Records. She was diligent in doing her “homework” to identify the most efficient and cost effect way to complete the project. Upon discovering, via consultation, with outside vendors the cost would be well over $18,000.00 dollars she immediately dismissed this option.
Maggie’s next step was to review options with the equipment that was available to her. Realizing this would be a long process she was able to create and operationalize a process that she believed would work and it did! Hundreds of Medical Records charts were separated, section by section, scanned and then organized into electronic files.
Throughout this long process, (which is ongoing), and in addition to her “regular” duties, Maggie manages the numerous challenges with an upbeat approach. Rather than see this project as “more work to do”; Maggie made it her goal to rise to the challenge! A monumental task which is why Maggie is so deserving on the Shining Star award.
