We are committed to providing compassionate supports and therapy to people facing mental health challenges in our Washington County Clinic or at our Bridgeville clinic. Job duties as a therapist include: Complete required evaluation and assessment for treatment of consumers, Complete all required documentation to include, but not limited to managed care information, initial consumer assessments, service plans, service plan reviews, progress notes, correspondence, transfer summaries and terminations summaries. Plan and coordinate treatment for consumers through participation in treatment team meetings, interdisciplinary team meetings, psychiatric staffing, and individual supervision. Develop and implement individualized treatment plans for an assigned consumers utilizing goal-setting techniques.
DAYLIGHT Direct Support Worker
As a Direct Support Worker in a day program with Chartiers Center you would be committed to providing daily living supports to adult individuals facing intellectual disabilities. Assisting Specialists in support activities for individuals designed to meet the individuals plans outlined in an interdisciplinary meeting and supports plan. Assists Specialists in carrying out individualized plans based on needs/wants/likes/dislikes of the individuals. Assist individuals at the program with personal care needs including toileting.
Mental Health Resident Specialist
As a Mental Health Residential Specialist in a residential setting with Chartiers Center you would be committed to providing daily living support to people facing mental health challenges. Job duties include: Develop, implement, monitor, and document Residential Service Plans for an assigned caseload of residents; Completing necessary skill training in the areas of daily living through hand over hand assistance, direct verbal and written presentation of information and observation, as assigned; Accurately complete all assigned observation, intervention, documentation of resident self-administration of medication, medication counts and necessary follow-up in accordance with Program policies and procedures; Effectively respond to, report and document all emergencies/crises/incidents in accordance to relevant policies, procedures and regulations.
